  11. 19________. I ,Randolph McCoy, being of sound mind, willfully and

  13. artificially prolonged under the circumstances set forth below  
  14. and declare that:

  16.     If at any time I should have an incurable injury, 
  17. disease, or illness certified to be a terminal condition by 
  18. two physicians who have personally examined me, one of whom 
  19. is my attending physician, and the physicians have determined  
  20. that my death will occur unless life-sustaining procedures are  
  21. used and if the application of life-sustaining procedures would  
  22. serve only to artificially prolong the dying process, I direct  
  23. that such procedures be withheld or withdrawn, and that I be  
  24. permitted to die naturally with only the administration of  
  25. medication, food or fluids or the performance of any medical  
  26. procedures deemed necessary to provide me with comfort care.

  28.     In the absence of my ability to give directions 
  29. regarding the use of such life-sustaining procedures, it is 
  30. my intention that this declaration shall be honored by my 
  31. family and physicians as the final expression of my legal right 
  32. to refuse medical or surgical treatment and accept the 
  33. consequences from such refusal.
